Every day, in New York State, thousands of families struggle to stay together as they face the stress of poverty, domestic violence, substance abuse, mental health, and unemployment. They have a desire to stay together but often lack the resources or support to do so- That’s where we come in.
Prevention Programs
Focus on the Future
This program works with youth and families to receive workforce education, develop community connections, learn life skills and get academic support. Sessions are designed specifically to address what the immediate needs are by providing youth and adults much needed life skills and resources so they can be independent and productive in their communities.
100%of the discharged youth maintained their educational enrollment.
Bronx Power Project
This is an after-school support program for adolescents living in the Bronx between the ages of 12 and 18 who have both substance misuse and psychiatric diagnoses. Referrals to this program come from Family Court, NYS Office of Children and Family Services, Legal Aid, various psychiatric hospitals, in patient and emergency rooms, schools and self-referrals.
100%of program youth will not become homeless due to mental health symptoms during enrollment.
Pathways
This program serves youth and families to prevent child abuse/neglect, recidivism back into foster care, and also service youth who have either been adjudicated a Person in Need of Supervision (PINS) or Juvenile Delinquent or are non-adjudicated youth who have been determined to be at “imminent risk” of being placed outside their home.
93%of youth remained successfully in their home without the need for out-of-home placement.

Family Connections
A short-term, intensive, home and community-based family preservation program. The program is designed to avert out of home placements of children at imminent risk due to juvenile delinquency, status offenses, child abuse or child neglect.
100%of the discharged youth maintained their educational enrollment.
Turnabout
A junior or middle high school-based truancy prevention program designed to avert out of home placements of children at-risk due to poor school attendance.
91%of turnabout youth had improved their attendance at the time of discharge.
Homerun
A school-based prevention program designed to avert out of home placements of at-risk children due to school-related issues and difficulties with family functioning.
96%of parents involved in the homerun program improved their involvement with their youth’s school.
Stepping Stones
An alternative to detention program which offers intensive crisis intervention. The program’s goal is to reduce the county’s need to place youth into detention facilities.
96%of youth remained in the home communities at the time of discharge and avoided the need for our of home placement.
Enhanced Stepping Stones
Stepping Stones is an intensive home and community based JD Diversion program. The program serves youth ages 15-17 impacted by Raise The Age legislation which includes all youth involved with the juvenile justice system both pre-adjudication and post-dispositional youth that are transitioning back to their home community and to reduce the risk of future contact with the juvenile just system.
95%of discharged youth improved their involvement in their educational program.
Vision Program
A school-community based prevention program designed to assist youth who are currently involved in PINS Diversion or adjudicated PINS. Upon county/school request, the program will accept Juvenile Delinquent diversion youth.
90%of youth improved their attendance while enrolled in the program.
Transitional Support
A family-focused program that assists children in returning from either residential placement or foster care placement to their family homes.
The Community Credible Messenger initiative Program (CCMI)
Our OCFS funded program that utilizes grassroots organizations to provide Credible Messengers and Parent Partners to youth who have been discharged from OCFS facilities to help the youth’s reintegration into their home community and avoid the need to return to placement. The program began in December of 2018 and will run through December 2023.
